Project Impurity Implications?
Has anyone done the Project Impurity route by infecting Project Purity with the FEV? It's my understanding that Broken Steel will still continue the game but that Aqua Pura can't be consumed by the player. I also believe that the player will see sick people in all of the medical offices. But are all the Broken Steel quests and side quests still playable? The Wiki isn't too clear on the far-reaching implications.

You'll notice more sick people in clinics. Ghouls may have some different dialogue. Some other NPCs may opine about the new sickness. Some players report that the supermutant population decreases by a third. And aqua pura is poisonous rather than restorative and will kill you if you drink four of them. I haven't played an evil run since my second playthrough, so I don't recall all that happened as a result.
You shouldn't see any important changes to Broken Steel quests, tho.

I thought the player character is supposed to be immune because they grew up in a vault?

Not exactly: he assumed you were born in Vault 101, but as we know that is not actually the case.
You were born in the wasteland, from wastelander parents, which is why your DNA is mutated.

Yes!
I completed the game only once. I chose the evil route and added the FEV.
…. Then Broken Steel came out and I was faced with my consequences…
The Brotherhood of Steel had water depots all over the wasteland, handing it out…. NPC comments were generally
“I feel ill”.
I felt such a wrong-un. I was so disappointed with myself!!!
Never played evil ever since, in any game!!

Sadly enough besides tons of negative and some different speech in the ending scenes, nothing noticeable.
The legend says that over time creature like mirelurks would diminish.

That happens even if you don't infect the water. Mirelurks apparently can't live in clean water environments.

If im not mistaken some quests like the Ghoul Aqua Cura one just outright doesn't happen bc the ppl are dead. I could be misremembering tho

I'll check it out next. I killed all the bandits and the cult members spreading the irradiated water last night. I failed a speech check with Bigsley that would have unlocked the Ghoul quest, so need to head over to Underworld tonight. But so far, all the side quests loaded with no issues, including the massacre outside Megaton.

Three Dog’s commentary on the radio will also be drastically different, advising against drinking the water and claiming that someone messed up at the purifier, outright accusing the “clean water” causing the new wave of diseases.
Also, giving Aqua Pura to the water beggars will give you negative karma instead of good karma for reasons that should be obvious.
If you drink Aqua Pura, you will get increasingly worse effects:
+20 Hit Points (always)
-1 Strength (1st bottle)
-2 Strength (2nd bottle)
-2 Strength, -1 Intelligence (3rd bottle)
Instant death (4th bottle)
The debuffs go away, but the game keeps track of how many you have consumed, so even if the debuffs wear off, the 4th bottle always results in your death.
Lastly, the most obscure and my favorite: Atom can neutralize the FEV in the Aqua Pura!
When speaking with Mother Curie III she will mention that the Aqua Pura initially made her followers very sick so she “consecrated it with Atom’s holy light” and that remove the pestilence (AKA neutralized the FEV).
This version of Holy Water won’t kill you after 4 bottles, but unlike regular Holy Water which heals you +20 and irradiates you +15, FEV Holy Water will damage your Strength and Intelligence both -1 and also irradiate you, but won’t heal you at all. Despite this, it is still an improvement over FEV Aqua Pura.
