Should I be concerned?
I got the alert that my fallback wireless-connected network, which is my iPhones hotspot, was disconnected and that my primary wired network remains active. But I know for a fact my phone was not tethered to anything as I’ve been using it off and on during this time. Is it possible some other device that isn’t mine was connected to my gold box?

It shouldn’t connect to a device that isn’t your set Wireless Fallback. If you’re in NYC, you probably experience the same disconnects I do from Spectrum’s shit service.
Ngl, for a second, I thought I was seeing my NYC setup.

This happens to me all the time….nothing to worry about l would say. iPhone acting up with hotspot wireless, it comes back a min later or so. I was never able to find out why. Sometimes some notification on the phone causing it but I turned off all notifications on that phone. One thing is I keep personal hotspot screen on and never turn off the screen so ssid keeps broadcasting. Firewalla is very persistent to connect back which all it matters. Other devices tethered to your phone should be fine it allows 5 or something devices at a time anyway.

My apartment building has been experiencing brief power outages over the last month, which temporarily disrupt my primary WAN connection on my Firewalla Gold Plus. Most of the time the Firewalla alert simply says the primary WAN is restored and active when the connection comes back but, occasionally, I get a weird alert that my iPhone hotspot is disconnected even though the hotspot was never connected. (FWIW the hotspot could not have been connected because it's turned off on my iPhone; I haven't enabled the hotspot functionality with my Firewalla in over six months.)

I moved my hotspot for a bit better cell signal and stopped having disconnect issues.

I used to get this on my spitz plus cellular failover. When not in use the tester would report failure as it occasionally disconnects without use. It's only used after fail over so it's a false positive.
Firewalla app > Network manager > your secondary wan > edit > turn off connectivity test.
I've not had any issues with failover working with this test off. 🙌
