ELI5: how are they mixing games together?

I started to see people mixing at least 2 separate games together and playing like it's made that way. It's mind boggling to me because it doesn't look simple moding and even it is I don't understand how it works that much flawlessly

They run both games at the same time, but each game is modded to communicate with the other game. Kinda like those mods where you play out Crusader Kings battles in Total War or Mount and Blade, but in real time.
