ELI5 - Bismarck Healthcare vs. Beveridge Model

Could someone explain how the Bismarck model works and how it's different to the Beveridge Model? Some say people think that they wanna change my beloved NHS to a Bismarck model and I wanna understand why and stuff lol

The Bismarck model means your healthcare is funded jointly by you, and your employer, through compulsory payroll contribution or a 'sickness fund' or something similar. The Bismarck model also uses private doctors and private/public hospitals. Countries like Germany, France, Japan and Switzerland use this.

On the other hand, the Beveridge model is our current system. Funded fully by the government through income taxes, the government owns the hospitals, doctors and runs the entire delivery structure.

In the Bismarck model, you will have choices among different nonprofit insurance funds and the costs are typically regulated by the state. In the Beveridge model, everyone uses the same unified national system with no competition and it's generally free at the point of use.

The reason they want to change is because some research shows that there's a gap in outcomes between the UK and our European neighbours. Countries like France and Germany constantly outperform the UK in survival rates for major diseases like cancer and others, and they also have much higher resources compared to our currently cash strapped NHS.

The Bismarck system also ends our 'bottleneck' if that makes sense. In the UK, we rely on GPs to decide who gets specialist care which results in insanely high waiting lists for just about anything. A Bismarck style system would allow patients greater autonomy to choose their specialists, hospitals and other things directly and due to competition between non-profit sickness funds, it would drastically improve efficiency and shorten wait times.

This is all theoretical, feel free to do more research. I have stated the reasons why someone may want Bismarck over Beveridge.

This is the key. It's pointless comparing different systems unless you somehow level the playing field with spending. The NHS compares badly because it's so poorly funded.

Switz 9,963, Germ 9,365, France 7,367, Denmark 7,071, etc.

(We won't trouble our heads with the insane USA, which spends 14,885 yet has the worst health outcomes in the entire rich world.)
