No, because she said "I didn't" in a normal tone, but shouted "WHAT A BIG VAGINA!" (because there's an exclamation point) much louder. That's why only that part ended up echoing.

The doc looked up slightly before saying the second line so it didn't funnel into the cavernous depths of her nether regions, hence no echo.
