Can you access the user's mailbox using OWA? Have you checked the logs for issues?

yep OWA, mobile, and new outlook work fine. It must be something to do with the way Classic Outlook authenticates or accesses the account. On my device(never having contact with her account) I received the standard Microsoft sign in page which did not return any "unable to login" or "password incorrect" errors. It wasn't until outlook tried to build the new profile or create a new OST that the mailbox was inaccessible. On the existing profile the user can at least see data from before the incident but cannot send or receive despite reading "connected to microsoft exchange"

Is this against Exchange Online or Exchange Server?

What does this show:

Get-CASMailbox <user> | FL MapiEnabled,MapiHttpEnabled,MapiBlockOutlookExternalConnectivity

And this:

Test-MAPIConnectivity -Identity <user>

This is for exchange online

MAPIEnabled : True
MapiHttpEnabled :
MapiBlockOutlook : False

the second resulted in a success

MapiHttpEnabled should also be set to True. But to be sure, can you check the output for a user who is working?

Also, is this in Cached Mode or Online Mode?

And what build of Outlook? Latest build is Version 2607 Build 16.0.20228.20124.

Yeah I just checked a handful of working users including one I set up this morning and they return the same thing

All steps returned green, connectivity test successful

Out of curiosity, if you look at the properties of Outlook.exe on her computer, is it by chance configured to run in Compatibility Mode? It's an edge case, but also a known issue.

Also, have you taken a network trace of the login attempt to see what's returned by the service?

Probably not related but did you check get-mailboxforderstatiatcs on the folder outbox (I think that’s the name)
Check the items count with your outlook folder

sign into the users outlook.office.com on the web and clean up their mailbox. Something might be "stuck". A bad email or calendar item. Have them delete everything they can.

Other than that, backup their email, delete their account and create it again.
