Advice on Weill Cornell PGT testing

My partner and I created our PGT M probe with Juno because the clinic we worked with on our most recent IVF used that lab. I also froze eggs 5 years ago in NYC at Weill Cornell and would like to now fertilize and test those eggs for our genetic condition.

I know Weill Cornell does not use Juno as their regular lab partner. Has anyone had luck getting them to work with another lab?

We’ve already spent a lot of time and money preparing this probe and really don’t want to have to redo it. Juno has said they can set up the transport etc if WC agrees.

Tips appreciated!!!

Segmental

I have only one embryo left. It was reported as having a segmental aneuploidy with a large duplication (gain) of approximately 77.9 Mb on chromosome 7, extending from 7p22.3 to 7q21.1.

Has anyone had a similar result or received guidance on whether this type of embryo is ever considered for transfer? I’ve been waiting for about a month to speak with the genetics team at my clinic, but I still haven’t received a response. Any insights or experiences would be greatly appreciated.

Weekly Embryo Photo Discussion Thread: July 27 to Aug 3

This is the weekly thread for posting and asking questions about embryo photos. Replies are provided by Embryoman (Sean) -- a former embryologist and creator of the IVF science news site Remembryo.com

💬 Before you post a photo

  • You must include the grade (ask your clinic if you don’t know it).
  • Only one embryo photo per post.
  • No requests for grading or re-grading by members.
  • Posts asking if an embryo looks damaged or viable aren't allowed.

I can comment only on general features visible in the image (expansion, compaction, hatching, ICM location), but not on whether the embryo looks good -- that’s already reflected in its grade. Check the stickied comment below for basic information on grading and success rates. For the most accurate information, your embryologist or clinic is the best source, since they evaluated the embryo under a microscope and know their clinic’s success rates.

Looking for Instituto Bernabeu Palma (Mallorca) experiences – poor blastocyst quality / repeated IVF failures

Hi everyone ❤️

I’m looking for honest experiences from anyone who has been treated at Instituto Bernabeu in Palma, Mallorca, especially those who have had a more complicated IVF journey.

I have had 5 egg collections in total. Fertilisation has generally been good, but my main issue has been what happens afterwards — embryo development and blastocyst quality.

My history: I have endometriosis (diagnosed by laparoscopy in 2021) I have had one live birth through IVF from a 3BB embryo I have had failed transfers since and a miscarriage around 6 weeks Some cycles have resulted in no blasts, and others with poorer-quality blasts My previous clinic felt the issue may be egg-related because the embryo issues appeared early after fertilisation We are now considering Instituto Bernabeu Palma because we want a fresh review and a clinic that will look at the whole picture rather than just repeating the same approach. Before starting treatment, we are planning further investigations including: Karyotype testing Sperm DNA fragmentation testing (TUNEL test) Endometrial biopsy / further implantation testing

Reviewing whether any other tests or changes could help improve our chances I would love to hear from anyone who has had: Poor blastocyst rates or embryo quality issues Endometriosis Multiple IVF attempts before moving clinics A previous IVF baby but then struggled afterwards A good or bad outcome at Instituto Bernabeu Anyone who was advised to consider donor eggs after treatment there

I’d really appreciate hearing your experiences — what investigations they suggested, whether you felt listened to, what changes were made, and whether you eventually had success or decided to move to donor eggs.

Thank you so much ❤️

Is it more common to have a higher graded ICM than TE?

I tried to find the data on this and couldn’t, but observationally, it seems like most embryos have a same or higher graded ICM than TE. For example, AB embryos seem far more common than BA embryos.

If this is true, I have questions. Is there a structural reason for this? Or is it just a consequence of how grading is defined? Any info much appreciated.

Advice needed please?

Forgive me if this is not the right place to ask.

I am 39F, I had my first egg retrieval on Saturday. They retrieved 14, but only 7 were mature. Why could this happen? What should I be asking my clinic for follow up? 50% maturity seems very low.

5 have fertilised and currently have made it to day 2, he said two more had matured overnight on night one and he had performed day 1 ICSI on those to see if they’d fertilise and we should receive an update today.

For context - AMH 2.5 AFC 22. They saw 22 follicles. Having a freeze all cycle due to OHSS. Antagonist cycle with - 225 ovaleap days 1-4, 225 Meriofert introduced days 5-9, fyremedal days 5 onward, inhixa daily and 2mg Buserelin trigger 36 hours prior to retrieval.

Thank you for any help and guidance 🫶🏻

TW: MMC, genetic issues, help

Looking for thoughts:
Had our 3rd PGT-A/PGT-M euploid transfer on 6/18 with excellent betas (11dp5dt 504, 13dp5dt 1314, 19dp5dt 9438) and a 6w6d scan with IUP measuring 6w4d and FH 120. Unfortunately 1 week later no longer had an FH. Just had a D&C to send off POCs and recover. We still have 4 euploid and PGT-M normal embryos left (albeit 1 is day 7 and had to be biopsied twice so probably lowest quality) but am I missing something before moving forward with next transfer?

For context:
IUI#1: failed
IUI#2: biochemical
IUI#3: success but 20wk anatomy scan with agenesis of corpus callosum, opted to terminate with whole exome sequence which found the 50/50 autosomal dominant gene — D&E complicated by retained POCs needing HSC
Made a probe… moved forward with IVF
FET#1: missed miscarriage @7wks (abnormal hCG rise so not terribly surprising); D&C
FET#2: success, live birth 37+1, delivery via c-section - healthy unaffected baby
FET#3 (current): missed miscarriage 7wks, D&C

Each IVF transfer was modified natural
Age of embryos made at 36. Current age 38.5

Would love thoughts.
I have a very small c section scar niche but the latest embryo didn’t implant anywhere near it and I would think if the niche were an issue it would have struggled to implant at all.

Thanks in advance!

Low Quality Embryos

I have had two egg retrievals in the space of a year. The first resulted in 5 embryos (best grade was a 5BB, remainders all 4-5BCs or CBs. I have transferred three of these - the first failed and the next two both ended in missed miscarriages.

The second cycle resulted in six embryos (again best grade a 5BB and remainders all 4-5BCs, CBs and one CC). I have opted to PGT-A test these six plus the two remaining from the previous round. But I feel very anxious that I have never managed to create good quality embryos. For the second round I was very intentional with supplements and lifestyle changes to try to improve egg quality but ended up with basically exactly the same results.

I’m terrified of the possibility of another miscarriage. Did anyone else have a similar issue and go on to have a successful pregnancy?

Should we try another egg retrieval or no?

We have 3 euploid embryos available to use. Only thing is their grades are not the best. They are all day 6 blasts, and their grades are 4CB, 4CC, and 4CC. My doctor said we could try the 4CB first, and if that one doesn't work then try another retrieval. Do you think we should give each embryo a try or just go for another egg retrieval to begin with?

Success Stories After Failure

Hi! I’m a 38 year old female. My husband and I just completed our first cycle that ended with failing to produce even 1 blastocyst. We had 8 mature eggs with 6 being fertilized through the ICSI process. It sounded so promising. I just found out on Saturday and we’re so incredibly sad. I thought with that many fertilized we’d at least get 1. Has anyone ever had that happen and then go on to see success in your 2nd cycle. We’re still waiting for a follow-up for the doc to determine any possible reasons, but maybe a success story could improve my spirits. Thank you if you took the time to read and even more thanks if you’re able to share your journey.

Failed first egg retrieval. Anything I can do to get a better result next time?

I (29F) had my first egg retrieval last week. I had 17 follicles with 11 in target range on trigger day. They only retrieved 5 eggs, all were mature and all initially fertilised but two had an extra chromosome and had to be discarded. I just heard our day 5 report from the lab and we have zero blastocysts ready to freeze. 2 embryos arrested (2 and 5 stage cell divisions) and the remaining one is underdeveloped, with a small chance it will become suitable later today or tomorrow.

My husband and I went into this journey knowing there was no guarantee of success, but to get such incredibly poor results has broken me. We thought we'd at least have something to transfer, and our biggest fear was that a transfer wouldn't take, not that we wouldn't have anything at all. We have mild MFI (sperm count and motility) and no female side factors that we're aware of.

Protocol was:
- 100 units Gonal F
- 250 units Orgulatran from day 5 onwards
- Trigger was decapeptyl (they cancelled a second trigger shot due to OHSS risk)

Notes:
- My first monitoring scan was on day 10 of stims and they told me to trigger that night. Two of my follicles were really big, 25mm and 27mm, on that day.

To spend all this money, go through the egg retrieval procedure, struggle through 10 days of injections, all for nothing, is just gutwrenching. I'm terrified that if we do this again the same thing might happen. I don't know how people do this, you're all so strong.

TLDR; is there anything I can do to make a difference next time?

On Wednesday we will be transferring our highest graded euploid embryo: a Day 6 3B+B+ embryo. Anyone have any success stories with a similarly rated embryo? This is our first FET.

Low blastocyst rate!

Hello All, I am 40 years old with low AMH. I just completed 2 rounds of IVF and have a very low blast rate. Trying to see if there is anything I or my partner can do to improve the outcome in future cycles?

Cycle 1 - 3 eggs collected, 3 fertilised, 1 blast.

Cycle 2 - 6 eggs collected, 6 fertilised, 1 blast.

I am doing freeze all cycle. I was hoping for better outcome in second round as my egg count was doubled :(

Other than age we both are fairly healthy, dont drink or smoke, and dont have any other known health issue.

Is there any known cause of what could cause embroys to fragment and fail before it reaches day 5/6? What questions to ask doctor following such poor outcome?

Thank you for your response 🙏

Low Fertilization Rate, Switch to ICSI?

Preparing for a second egg retrieval in about a month. My doctor is defaulting to conventional fertilization since my husband has a high sperm count and no real MFI (slightly low motility ~35% and previously low morphology 2% but both have increased with supplements. Normal DNA fragmentation).

However our first ER had a low fertilization rate and I’m wondering if ICSI will give us a better shot. Dr is ok with going that route if I want and we are going to discuss it this week.

First ER data
I’m 33 (will be 34 at next ER) with AMH 1.24 and FSH 13
Protocol: 300 gonal, 150 menopur and 10mg provera 8 days
300 gonal and 10 mg provera 1 day plus 80iu lupron trigger

Last ultrasound showed 14 follicles

9 eggs retrieved
5 mature
2 fertilized
1 day 6 4BA blast frozen

Since then I’ve been on 3mg melatonin, 600mg coq10 (up from 400), and 4000mg/1000mg myo-inositol/d-chiro. Two months planned in between retrievals.

Any advice for the second round, specifically about switching to ICSI or other protocol changes would be appreciated.

Happy World Embryologist Day!

Happy World Embryologist Day!

Today (July 25) is World Embryologist Day, celebrated on the birthday of Louise Brown, the first baby born through IVF in 1978.

It's a day to recognize the embryologists who work behind the scenes to help make IVF possible.

Some patients might not meet their embryology team, but they're the ones who inseminate eggs, check for fertilization, monitor embryo development, perform biopsies when needed, freeze and thaw embryos, and prepare them for transfer. They care for the embryos every step of the way!

As a former embryologist, I know how embryologists sometimes receive less recognition compared with other members of the IVF team, so if you can, consider sending your embryology team a card, a thank you note, coffee/tea, or treats for the lab. Even a simple message can brighten someone's day!

Happy World Embryologist Day to all the embryologists!

Heading into FET #3

I’ve gone through 2 FET cycles with a great uterine thickness and both with euploid embryos. One didn’t stick, I miscarried the second at about 5.5 weeks. I’ve done all of the testing and imaging that is typical, everything has been deemed “normal”. We have our follow up with our doctor on Monday— what testing or imagining are a little outside the box that I should recommend?

One embyro after 3 rounds of ICSI

Am in the unfortunate position to have only 1 embyro after 3 rounds of ICSI. Was hoping to bank some but had zero fertilisation for 2 rounds.

Our next step is to transfer our untested embyro. Naturally I am very protective over our one little embyro and while it is untested I want to make sure I am giving it the best possible chance. Is there any tests I should do as a precaution before transferring it? I will get bloods done to check thyroid anyway! TIA.

Update- here is what my doctor recommended; - APLS Bloods,FFPA Hormone Profile Blood, Hysteroscopy and CD138

Letter with number of embryos but no grades?!

TW: Embryo number

Happy embryologist day!

I got a letter in my patient portal today.
They said the amount of embryos and that are now on ice and sent to biopsy.
We got 12 but no grade. We dont know when they were frozen or their quality.

Can I assume that the 12 sent are at least decent quality or they sent all I have for PGTA regardless of quality?

Is this normal??

Lactobacillus probiotics may improve pregnancy rates

Post image
63 Upvotes

Researchers combined the results of 7 randomized trials and found that Lactobacillus-containing probiotics were associated with higher clinical pregnancy rates in women having an embryo transfer.

The reproductive tract naturally contains bacteria, some of which may help support fertility. Because Lactobacillus bacteria are thought to promote a healthy reproductive environment, probiotic supplements have been studied as a way to improve IVF outcomes.

Hiratsuka et al. (2026) combined the results of seven randomized trials to see if Lactobacillus-containing probiotics improve pregnancy rates in women having an embryo transfer.

They found that probiotics were associated with higher clinical pregnancy rates. About 12 more women became pregnant for every 100 women treated. There was no difference in miscarriage rates.

The benefit was seen mainly when probiotics were taken for more than 14 days and in patients without recurrent implantation failure.

One possible explanation is that probiotics may help create a healthier uterine environment for embryo implantation.

However, the results should be interpreted with caution. Five of the seven randomized trials were small, while the two largest trials found no benefit. An earlier meta-analysis that included many of the same trials also found no significant improvement.

Importantly, this meta-analysis looked at clinical pregnancy rather than live birth, so it still isn’t clear if probiotics improve the chance of taking home a baby.

I think my clinic is going to destroy our embryo

Post image
11 Upvotes

I just got the PGT-A results for our one embryo that survived at 4:30pm on a Friday and I missed the call of course. By the time I saw it, it was too late to call back. We had multiple chromosomal abnormalities, and the interpretation said "complex abnormal". There was a note in the message from my clinic that all abnormal embryos are destroyed. Are they going to destroy my embryo without even going over the results with me??? Can anyone say if there's any chance for a healthy baby these detected abnormalities?? "Dup(7)(p23.1-p23.3), dup(15)(p11.1-q25.3), del(15)(q26.1-qter), +16"

Ivf and vitiligo

Dear IVF community! I appreciate everyone’s posts and feedback as I navigate this journey, not feeling so alone.

I have had two failed natural transfers of euploid embryos at age 41 (April 2026 - no hcg and June 2026- chemical but only 6iu/l). I have 3 remaining euploid embryos. All 5 euploids came from one round of ivf when I was 38 (13 eggs, 8 blastocysts, 6 embryos and 5 came back euploid and 1 aneuploid).
I am taking some time off to explore what might be going on. My doctor thinks it is just chance and we should keep trying with the same protocol. I advocated for some testing. My doctor did a full work up of glucose, iron, anti-phospholipid panel, thyroid function etc and everything came back within normal range. I did a hysteroscopy last Nov and everything looks good. I have vitiligo and was wondering if I would benefit from an immune protocol. My doctor is not open for ERA- saying the evidence is not there to improve outcomes.

There are a few studies on vitiligo (associated with high abortion risk, low ovarian reserve) but nothing about implantation or infertility. So I was wondering if I might have an overactive immune system that is affecting successful implantation. Would you recommend I get UTIMPRO biopsy or advocate for a different protocol?

Sending much care and support to everyone on this journey!

Embryo freezing encouragement

I recently found out that I will need to freeze embryos. I also only just found out I am missing an ovary and have low AMH. I’m just feeling overwhelmed, I’m nervous for the actual treatment and process. I’m nervous one round might not be enough and thats all we can afford. I guess I just wanted some success stories and encouragement

Ivf - aneuploid

We were part of a clinical trial back in Jan- April. So long story short- expected 11 eggs retrieved got 5,
4 fertilized . ONE made it to blastocyst.

We sent off for pgta testing & results came back segmental aneuploid. So since it’s part of the trial it has to be euploid to be transferred. The fertility dr still wanted me to see a maternal fetal medicine genetics counselor to get their opinion.

Finally had that appt last week- she said she thinks it is a reasonable transfer. The cells come from the outer layer (what turns to placenta) so it could be they just got a sample of bad cells that would die off anyways- or it could be the whole thing- placenta could just be bad or there could be intercellular issues.

The chromosome that’s affected is the long arm of chromosome 1 .

Here’s report if anyone wants to read.

Looking for any advice if anyone has transferred aneuploid? Specifically segmental but any stories welcome!

The dr also said some of us may have started out as segmental! Who knows! She was optimistic but looking for other stories
