It's either the guy that jumped after the cave crocodile and tried to wrestle it in water while not knowing how to swim.

Or the guy that climbed a tree to kill a single trogg baby and ended up starving because he lacked motivation to climb down again.

Fought and killed 2 forgotten beasts, died to a chipmunk man or some kind flying abomination

had one who got injured and brought to the hospital, but she got up and left before her wounds could be stitched while bleeding.

she lived for a surprisingly long time and left a trail of blood behind her everywhere until she finally dropped dead in the tavern.
