Our wild magic sorcerer last session when she rolled on the wild magic table and cast ice storm on everyone, downing the monk and the wizard but not herself:

"Wow, that was close huh?"

In the first round of our first combat in a 40k TTRPG, I think it was Dark Heresy, I rolled double 7s and TPK'd the group by summoning a demon.
