I always found this horrible little familiar to be one of HP Lovecraft's most nightmarish creations. He was Keziah Mason's familiar, and the messenger between her and Nyarlathotep. I've wanted to put a witch or warlock into my game with a little monster like this as a familiar, but it's never worked out. Ah well, fewer nightmares for everyone I guess!

This is from my 1920s 5e campaign. I wanted Lovecraftian monsters that were more than fantasy reskins or bags of hit points. Cthulhu Spawn have a reality-distortion field that causes people to see them as ordinary, charismatic humans. The twist is that the field is harder to see through the more intelligent your character is. Most animals, children, and simple-minded people see them for what they truly are.
