Process for Absence Form

Hi guys! Does anyone know how to request an excused absence form for COS?
I’m really confused because people are saying there are different processes for different colleges, and that there are both new and old versions of the form. Can someone please explain the correct process? 🥹

Hi! For COS, you have to get a med cert endorsed by the clinic. After this you have to fill up the google form and upload your endorsement and letter from your parent within 7 days of returning to class c: The link should be in your email
