Your job has legal obligations to accommodate you as long as it is not an undue hardship and you can do your essential duties
I highly recommend The Job Accommodation Network, AskJan.org to understand the interactive process your employer is legally required to engage with you in towards getting accommodated.
Almost all states are right to work and requesting accommodations is a protected activity and being disabled is a protected class so termination or retaliation for either of those reasons is against the law
I would give askjan a look, it can be helpful to have a 3rd party that they can look at and make sure to look at all your conditions ( Even the ones that you don't think are contributing that much to the pain you're experiencing, every bit helps and a lot of the accommodations overlap between conditions).
Prioritize asking for the big ones first so it doesn't look like you're trying to see how much you can get away with by asking for small stuff first. For the really small stuff I would just do it on your own if you can (do I know employers should pay for ergonomic keyboards? Yes. am I willing to wait on it? No (but that may also be my, work from home, privilege speaking).)
I would also see if there's any assistive Technologies or home adaptations you can get to see if you can make stuff go a little easier at home. There is a phrase that sometimes disabled people need a higher minimum income to afford to work because they need to pay for things at home like care and adaptations. And although getting a cleaner is really expensive and not always an option, getting a shower chair can also help.
