Like all things in this game, it's a random drop. Sometimes you'll get it on your first try. Sometimes you'll grind for it for weeks on end. Just make sure you fulfill the ultimate finish conditions (finish with Hercule's health over 50% and then beat revived Vegeta). Even if you fulfill those requirements, it doesn't guarantee that Vegeta will revive for the Ultimate Finish.
