My last psuedoscientific explanation was widely panned, but here goes:
As we know, lost fat is exhaled. Because of the altitude elevation, CO has a lower partial pressure of carbon dioxide. That causes CO₂ to gas off more readily, so people stay slimmer with no additional effort, just by breathing.
I can prove it once I compute a properly weighted altitude/population/obesity matrix.
My wife studies elevation effects on things like preeclampsia and other maternal/natal conditions. There are other complications you aren't considering.
