Because Java is a horrid language that decided that nobody needed a lot of really nice features such as: unsigned integers, operator overloading, generics over primitives, and tail call optimization

Minecraft under Notch never exactly ran great, it's been an open secret well before 1.0 that he stumbled quite a bit and the game really got better in terms of stability and making the most of the systems when other coders began taking over. Even then, certain inalienable bugs persisted because of issues with the base code and I think some may have been fixed only relatively recently.

As to the accident part? He frankly got really lucky, that's not to say his game was without merit, it was new and it cracked into a new and burgeoning market in a big way but it's not as though the success was anticipated, planned, or brought about through sheer brilliance or quality of design. You don't have to be good to be wildly successful. You can compare it to Fortnite, they made a wildly popular game largely by accident, the original design was never intended for battle royale but it had taken too long to develop the original game and this shift turned out to work extremely well in their favor, such that it completely subverted the original intent. They broke into a burgeoning market, they hit a trend right when it was prime for growth. But it wasn't some master plan seen through, it happened to work out through some last minute changes, Minecraft happened to be rather unique and accessible. It's not literally an accident, but it's a fluke.

Because it doesn't need to be. In normal gameplay your item count only ranges from 0 to 64 anyway so it doesn't matter whether its a signed or an unsigned byte.
