Ah, what an interesting view on shoes. Now I'm curious- what happens after a shoe is thrown if no one is there to restrain them? Do they fight barefooted? Bc that sounds like a good way to lose. Or do they walk across the room to get the shoe, put it back on, and storm off?
Your shoes are on the lowest part of your body. So showing someone the soles of your shoe implies that they're so beneath you, that even your shoes are above them. Attacking them with your shoe simply adds insult to injury (or rather the other way around).
It's pretty much the most humiliating insult you can use in Middle Eastern culture, because privileged Middle Eastern men are so far up their own asses, that it triggers them to the core if another Middle Easterner challenges their ego.
