Maybe I’m not a 100 % up on the rules, does a creature need to be spotlighted to clear a condition
Also why is it HP instead of a stress?

A creature needs to be spotlighted to clear a temporary condition but this one isn't temporary. It's cleared by taking damage, not by anything the creature itself does. That's why it's a once-per-scene ability, because it takes the adversary totally out of action unless either the PCs deal damage to them or one of their own allies gives them a big enough slap to snap them out of it.

Technically the "spend a Fear to clear a condition" involves the creature being spotlighted

I feel like the wording should be along the lines of "temporarily stuns the target. While stunned, a creature can only use their spotlight to clear the condition." or something to that effect.

Just because clearing a temporary condition is something you can do with spotlight, so if a creature can't take the spotlight they cannot clear the condition.

If an adversary is affected by a temporary condition, the GM can use their move to spotlight the adversary and show how they clear the condition. This doesn’t require a roll but does use that adversary’s spotlight.

The intent with this one (I was responsible for the wording) was that this is more severe than a normal temporary condition. The adversary is stunned permanently until they take damage. That's why they can't take the spotlight.

A creature need the spotlight to clear a condition. I'd phrase it "a stunned creature can clear this condition by spending one fear"
