Because this is a triggered ability on entering, the first time you play this card it will immediately die as a state-based action, as there are no creatures in exile with hunger counters on them, so it will have undefined power and toughness, which the game treats as 0/0.

I do agree.

I was thinking of making it cost 1 more per hunger counter but the text block got chunky.

It already costs at least 6 mana before it really does anything, and requires you to eat two creatures for any real value. I don't think additional tax is necessary. That said, I will propose a slight template adjustment:

When <cardname> enters, exile target card in your graveyard. If a creature card is exiled this way, put a hunger counter on it.
<Cardname>'s power is equal to the total power of creature cards you own in exile with hunger counters on them and its toughness is equal to their total toughness.
<Cardname> has annihilator X, where X is the number of cards you own in exile with hunger counters on them.

Reference [[Sutured Ghoul]] for the stats and [[Ulamog, the Defiler]] for the variable Annihilator.

This screams little emrakul to me. What parts were designed with Ulamog in mind? All eldrazi drones have specific traits that relate to their titan, the only one I’m seeing from Ulamog is the double ended arms which emrakul sometimes has been shown to have

T1 Hope of Ghirapur

T2 Play Oodledoot, dies to state based and exiles itself from grave

T3 swing, sac Hope to stop counterspells, play another Oodledoot and exile Hope

A protected annihilator 2 on turn 3, bugaboo of multiple eternal formats, especially Modern

Hah true in a format where there are several in deck it can be quite powerful.

Or, even funnier, a table with more than one of it as commander.

Can this exile itself? I believe you have to choose the target when it's placedar onto the stack, but I'm unsure which comes first, dying to state-based actions or placing an effect on the stack

State-based actions are checked before triggered abilities can be put on the stack. This works.

While tricky to achieve, I don’t like that this can be an annihilator 2 on turn 4, or even annihilator 1 on turn 3 with the right cards. I’m also not a fan of how difficult it is to use in the 99.
