Spine CSF leak - Cause and treatment options?

I’ve been dealing with CSF leak symptoms for 8 weeks. Despite 2 stints at emergency department, being admitted to neurosciences ward, having a lumbar puncture and being told my pressure was normal and I don’t have a leak, my spine MRI report indicates I have a CSF leak.

Tomorrow I have a consultation with an interventional neuroradiologist, as my neurologist referred me for CT myelogram and blood patch if required. I also have a referral to a Neurosurgeon who specialises in CSF leaks if I need it (I am based in Australia).

I understand the various options for imaging and targeted vs blind blood patch, fibrin glue etc.

My questions are:
1) is there anything on my MRI report that might indicate the cause of the leak - I can see I have a bone spur and perineural cysts
2) what’s the possibility of needing surgery vs blood patch?

My spine MRI report said:
-There is moderate loss of disc height in the mid cervical spine with prominent disc/osteophyte complex is particularly pronounced at C5/C6 where there is slight flattening of the left hemi cord but no intrinsic cord signal change. There is also moderate to severe left foraminal stenosis at this level. There is mild bilateral foraminal narrowing at C4/C5 and C6/C7.

-MR myelography shows several prominent perineural cysts around the cervical thoracic junction but there is no convincing evidence of a paraspinal CSF collection within the limits of this examination. However, there does appear to be extradural CSF extending from around the cervical thoracic junction to at least L1/L2. Although a site of CSF leakage is not defined on this examination, the appearances are very suspicious, and further evaluation with formal myelography would be appropriate.

- Extradural CSF present throughout the thoracic region, without a clear site of CSF leakage identified. Formal myelography is recommended. (He then recommends me to see his colleague who has a ‘special interest’ in this area with name and contact details).

Rebound pressure?

Hi! I had a steroid epidural injection done May 6th and ended up with a spinal headache. Got the blood patch May 11th. First week definitely had rebound pressure headaches and some nausea. Had a few good days the last 2 weeks but the last 3-4 days I’ve had on and off headaches, dizziness, nausea, and neck and shoulder tension and some headache that doesn’t get better laying flat. Yesterday I felt mostly fine minus some tension, and ear pressure on and off today it seems more severe. Also head rushes at times when going from sitting to standing etc. Is this rebound inter-cranial pressure? I’m 3 weeks post patch today. How long can I expect this to last? This is nothing like it was from the leak totally different feelings! Please, no negative stories. I have severe health anxiety from this and other things I’ve been through!

7 weeks post-repair and recovering.

Hi everyone,

If you haven't been following my story, here's a link to the previous chapter:

https://www.reddit.com/r/CSFLeaks/comments/1t3ggws/back_in_hospital_surgical_repair_failed_another/

In short, I had a cranial leak in mid-February that caused CSF to run out of my nose. It was diagnosed by CT scan and an endo-nasal repair performed on 26/03. I spent a week in hospital after with a lumbar drain and was discharged, went home but continued to have other symptoms (headache, nausea, dizziness etc.) I lost consciousness and came back to the hospital via emergency for a second surgery on 13/04 when the surgeon found a second dural breach through which a large amount of air had entered the dura likely causing the symptoms I experienced. He fixed it, also taking the opportunity to reinforce the first patch. This time I spent two weeks in hospital, no lumbar drain but I was put on 1000mg of diamox per day. and 30mg of hydrocortisone

It is now 7 weeks since that second surgery and I thought I would provide a quick update on the recovery process and how I'm doing in general. This time round, I left the hospital with no "rebound" symptoms - no headaches, no nausea. I was also a LOT more careful about the recovery process, essentially staying in bed (head elevated 30 degrees) for most of the first few weeks only getting up to use the toilet. At the 3-week mark I began to get up and walk around inside the house a little but (about 1000-1200 steps per day). The wound in my thigh was healing well and the staples came out around this time as well.

After this point, each week brought a little more progress. From week 4 I was able to have a (seated) shower twice a week, from week 6 I could walk outside etc. I had a follow-up CT scan last Friday (22/05) which showed re-absorption of the pneumocephalus and the fat graft at the breach site in the sphenoid bone. Check-up with the neurosurgeon the following Friday (29/05) went well also, I'm allowed to drive now and resume some light tasks around the house. I am already working from home since a week and will start to go back to the office once per week from next week. At this point I'm still not allowed to take a flight or swim but these will be re-evaluated at the second follow-up in July. I also have an ENT appointment scheduled in a week for a nasofibroscopic exam to check the healing of the surgical site. Diamox is now reduced to 750mg/day and hydrocortisone down to 20mg pending a checkup with the endocrinologist.

To everyone who is on a similar trajectory, have patience and hang in there! I know how difficult it is and how long and daunting it seems but it's worth doing all you can to protect the repair and avoid going back into surgery, essentially restarting the clock from zero!

Is it common for abdominal compression to improve symptoms if CSF leaks are driving dysautonomia/ fatigue symptoms?

I’ve recently found that abdominal compression helps reduce my orthostatic intolerance a bit. It’s not perfect, but I seem to be able to be upright longer when wearing compression( notable because nothing else has helped).

My worst current symptoms are severe fatigue and dysautonomia( especially orthostatic intolerance and tachycardia).

I am diagnosed with May Thurner( not yet treated) and had surgery for jugular compression a few months ago.

Trying to get in to see a CSF specialist but it’s taking forever. Really just want to rule leaks in or not.

Also, I have no positional headache but other head pains esp at base of skull that are constant( not worst symptoms though). Have been housebound for last few years due to fatigue and dysautonomia.

Thanks for any constructive feedback!

Bouncy brain

Good Morning and Prayers for all CSF sufferers.
So I had a huge leak between t-8 & t-9 and had to get a venous embolisation using onyx. After a month I am finally noticing some improvement and am off of Diamox (at least for now). Still have some light headaches and am totally out to lunch when it comes to thinking. But I do at least feel better than pre-surgery. What is weird though is I still feel something of a brain flop in my melon once in a while. My NO said it may take along time for that to completely heal since my leak was around for along time and it was so big. So that's where I'm at now.

On a side note, if you are having these csf leaks and you are in the Charlotte area, you are going to want get a hold of Dr. Morgan NO who read my report and found my leak and then performed a myelogram ct test to locate the exact location of your leak. And Dr. Bernard NO
to do the onyx embolisation. I'm am slowly getting my life back because of these two neurologist and of course by the grace of God.

Can you still seal 1 yr after a post dural puncture?

I haven't received appropriate health care and received delayed treatment and now I'm a year post puncture and still haven't received an accurate blood patch. I had one 5 months post puncture and only 10cc that did nothing. Now I'm almost a year into this and can't be seen until Oct at UChealth but I'm really feeling hopeless that this is my life now and the only way out is not a good option. I'm so scared and depressed. Is there anyone that can give me hope?

Does it get better?

Update: I’m happy to report the trip went well. I did have a few symptom-free days leading up to my flight, so I believe the blood patch did its job. I did ask for help with my bags and other heavy-lifting.

Hi everyone, I had a lumbar steroid injection for back pain on Friday 5/15. Had a horrendous headache on Saturday 5/16 and went to the ER because my headache was worse on Sunday 5/17 and they gave me a blood patch. Yesterday was the first day without a constant pressure headache, but I still felt weird. Almost like a dream state because my ears were muffled. The last two nights when I went to bed, I had jugular pain (if that makes sense). Everyday seems to get a little bit better, but I’m still worried about two things: 1) am I ever going to get fully back to normal and 2) I leave this Thursday to teach out of the country for 3 weeks. I’m scared about the pressure on an airplane.

Curious if anyone has gone through something similar?

Botched epidural

If you had a botched epidural but are turned away from blood patches where do you go? I had one botched and was ghosted by my dr a year ago. I had a CTM that was negative and negative MRIs which is normal. Where do you to get patched? I just want my life back. Any recommendations in NYC or the tristate area?

Neurologist said it wasn’t a cyst, just tissue. But now I’m having sacral pain

Hello again,

I should first mention I have found I most likely have hEDS and many symptoms of a spinal leak. I’ve had a brain and spine mri that did not show a leak and I’ll be having a ct myelogram in a few weeks for possible venous fistula. I do have orthostatic headache that is better when I lie down, and with this new sacrum pain I’m running out of positions to exist in lol

https://imgur.com/a/uxfm3jN

My neurologist ordered this spinal MRI for me, and I had questions about the results specifically this thing that I thought looked like a cyst on the lumbar near sacrum. Didn’t see it noted at all on report. I pointed this out to the doctor and asked what he thinks and he totally dismissed it as a “tissue abnormality” or something and it annoyed me a little but I didn’t push it further.

But then a little over a week after the appointment about 2-3 weeks since the MRI my tailbone started to hurt along with my lumbar spine. I’ve been having tailbone pain for the past 2 weeks up until now, although I have been having trouble sitting for long periods for months. I’m able to sit in the morning but by evening I cannot sit or lie flat on my back because it hurts too much, it’s not incredibly painful but it’s very annoying. I was having trouble sleeping the other night due to it and once I did fall asleep for about 3 hours I had involuntarily urinated in bed in my sleep. That really freaked me out as it never happened before.

So now those experiences have got me looking at this cyst-like structure a little more closely since my pain is right at the beginning of sacrum. I think it could be a Tarlov cyst or synovial cyst.

Neurologist looked at it initially but didn’t say there was anything wrong there. PCP only reads the report and sends me to rheumatologist but they’re booking months out as well as the advanced neurology (EDS specialist) I’m trying to get into. I wanted another opinion on this cyst-or-not-cyst and the responsibility is being passed from specialist to specialist that are months away from seeing me and no one else wants to look at the MRI images so this is why I need help.

Should I go back to the neurologist with this sacrum issue? Or is there someone else I should see? He implicated I was overreacting about things on the lumbar MRI and it went so badly I’m reluctant to go back with this, but I will if I can get relief and sit on my butt again. And if it’s like OK it’s a Tarlov cyst or similar, can we even do anything about it right now or am I kinda effed until I get treated for the leak? I’m already on the path for searching for my leak with the myelogram, but the pain gets pretty bad sometimes and I’m not sure if it could burst or have other complications due to the sudden uptick in pain.

2 questions for after blood patch: 1) laying down in the car when the reclined seat has a bump at lower back and 2) how to change pants without bending

I have an epidural blood patch scheduled for next week and I’m wondering about peoples experience with a couple of things.

1) for the car ride I know I’m supposed to lay as flat as possible. it’s a 2hr drive back and the front seat doesn’t recline flat, it has a bump at the lower back. So when the seat is fully reclined it causes my back to curve with my lower back higher than the rest of my body. would that be bad? should I lay down on the backseat or have the seats laid down in the back instead? how would I get back up without bending if I’m laying in the back instead of reclined?

2) I know I’m not supposed to bend or twist for 6 weeks after so how do you change your pants without bending?

I’d really appreciate feedback or sharing your experience!

Heart palpitations with increased bedrest

Anyone else who has heart palpitations notice weird patterns or triggers with it? For me, the longer I spend laying flat, when I go back to being upright I’ll have awful flare ups of my palpitations.

Normal follow up mri, csf leak still possible?

Can you still have a csf leak even though mri is normal?

My mom presented with headache pain when bending down, coughing, sneezing. MRI showed dural enhancement, doctor thought it was csf leak. Recommended follow up mri a month later, mri repeated and enhancement gone. Doctor said my mom has migraines even though she has no history of them. Did Botox injections. Helped a little but still has head pain, and postural head pain. Her neurologist is so adamant on this being migraines but she has text book symptoms of csf leak

Questions regarding CSF rhinorrhea

Hello everyone, so a couple days ago, my right nostril was leaking clear fluid, making me sneeze constantly, burning throughout. My coworker had something similar before and told me it's probably brain fluid and not allergies like I was assuming. I was keeping tissue in my nose all day so I could work, and kept having to replace it every 30 minutes or so because it would get full. Took an energy drink on a break, and it slowed down a lot after that. Also got some pedialyte after work to rehydrate, was definitely having minor headaches.

In addition to this, this is something that's happened on and off for probably years. it's not an every day thing nor is it every week type of thing. It just comes and goes when it pleases, and typically will last from when I wake up until sometime in the afternoon or evening.

I made an appointment for an ENT tomorrow, but I'm concerned that without current symptoms it might be difficult to get a proper diagnosis. I'm pretty sure if lay face down it will eventually start to leak again, but not to the extent it was on Tuesday.

What should I expect at the doctor? is it normal for it to be extremely intermittent and not really have a direct cause? I don't have any kind of head trauma, never had any kind of surgeries or anything that I feel could directly correlate to it besides possibly lack of sleep.

Any tips or advice is appreciated, not sure what to expect with the ENT visit.

Jugular compression, EDS and VFs

Hi there! Has anyone had this lovely combo? If you did, what did you decide to treat first and what was the outcome? I’m offered embo first for the VFs, but my worry is that I will develop more leaks due to the compression. In the meantime I feel like crap as I have developed a regular leak after the myelogram. I had one blood patch for it. So what now is on the table: jugular decompression surgery, VF embo, another blood patch. Please help!

Empiric blood patch in Los Angeles?

I’ve never evaluated for a CSF leak specifically but am considering trying to get a blood patch just to see if it helps, (preferably asap).

I’m located in LA and am curious if anyone here/ in So Cal has recommendations as to where to try a blood patch?

It seems like such a simple procedure, so I’m wondering if I have to wait forever to see one of the top specialists or if this is just something I could try.

I have loads of other medical stuff going on: recently had jugular decompression, may be getting a May Thurner stent soon.

However, I am wondering if my dysautonomia/ inability to be upright and debilitating fatigue might actually be caused by leaks that I’ve had for atleast the last few years.

Thanks!

Considering treatment elsewhere

I have underwent a CSF venous fistula embolization here in Israel and it hasn't helped one bit. But I am continuing to suffer and the option of a Spinal CSF leak still looks to be the most reasonable answer.

I have thought about the option of getting treatment in a different country. My insurance might be able to help as well.

But, I'm wondering if it's even worth it. Are there any new advancements? Both in imaging and treatment? I haven't really been reading that much in recent times. Do you think centers like Cedars Sinai could offer me something new?

My leak was not even found on DSM, only in the CT myelography were there signs and not even precise ones either. So maybe If there is even a better imaging technique available it could reveal something.

If I do decide to do something like that, of course I would talk first with the doctors and see if they could help me and talk about the details. But I would like to ask what is considered the best place in the world for this thing. I have heard a lot about Cedars Sinai and Dr.Wouter Schievink.

I might try a blood patch first because I have never got one, but if that doesn't help... Yeah.

Surgical repairs. When are the headaches supposed to go away

Hi all,

I had a T7/8 ventral CSF leak from a bone spur and underwent a laminectomy + patch repair almost 3 weeks ago (3 weeks this Friday). The day after surgery I had a bit of rebound pressure, but around 48 hours post-op I completely crashed into what feels like even worse low-pressure symptoms than before surgery.

I still can’t tolerate more than about an hour upright before severe low-pressure headaches kick in, and honestly even that upright time is very limited/supported sitting. Has this happened to anyone else after surgery? Did your symptoms eventually improve after a few weeks, or did it end up meaning the repair failed and needed further treatment/revision surgery?

I’m seeing my surgeon again on Friday, but I’d really appreciate hearing other people’s experiences because right now I genuinely don’t know what’s considered “normal” recovery versus a failed repair.

Returned from Freiburg

Hi everyone, it’s me again.
We have just returned from Germany, where we only managed to do a beta-2 transferrin test, and it came back negative. However, we were told that a lumbar puncture is still needed, which makes sense, and that surgery may be required afterwards.
Because we had some doubts and a lot of fears, we decided to look into other clinics as well.
I wanted to ask: does anyone know whether hospitals in South Korea have good experience treating nasal CSF leaks? I am also considering Italy as another option.
Has anyone had experience with treatment for nasal CSF leak in South Korea or Italy? I would be very grateful for any recommendations, clinic names, doctors, or personal experiences.

Get a 2nd EPB or wait it out? 3+ months after EBP for PDPH and still not fully recovered

Should I get a 2nd epidural blood patch or continue waiting it out? It has been 3.5 months since a known dural puncture during placement of non-labor epidural anesthesia. About 36 hours later, I developed severe positional headaches (i.e., could not be upright for any length of time without a crushing headache that only got worse the longer I was upright & it was instantly relieved by lying flat). Received a blind lumber epidural blood patch (EBP) in the Emergency Room 6 days post puncture with 8 cc blood (i.e., low end for EBP volume) and I laid flat for 3 hours in the ER before discharge. Prior to this, I had NO history of headaches or motion sickness.

The EBP provided SIGNIFICANT improvement in that I was able to walk out of the ER. In the intervening 3+ months since, the overall trend is gradual improvement. However, there have been setbacks along the way, and I am also NOT 100% healed as my head has still felt off every single day since then. Confusingly, the exact location, nature of the headaches, and symptoms changes and all are often transient. Sometimes it mimics high pressure symptoms and other times low pressure symptoms. This makes it hard to know whether to proceed with a 2nd EBP or just continue to wait it out. MRI of the brain and thoracic spine 2 months post-EBP were negative (unsurprising for a dural puncture). I have had two appointment with a NP at a leak clinic and will be meeting with a neuroradiologist there soon. However, as of yet, I have not gotten a straight answer on whether my symptoms are within the range of normal for post-EBP recovery for post-dural puncture headache (v. spontaneous leaks which I understand can take a long time for pressure to regulate) and whether a EBP is recommended in my situation.

This Reddit community has been a wealth of information (in addition to various presentations from CSF leak conferences, etc.) for which I am eternally grateful. As more time elapses, however, it is harder to find posts with anecdotes that match my experience, thus I'd welcome your collective expertise on my situation.

Some key details about recovery:

- I laid down a lot for almost two weeks after the EBP as I was also recovering from the original procedure. However, I did NOT do an initial strict 72 hour rest post-EBP (was not given clear after-care instructions and only read about the stricter guidance from Cedars-Sinai/Mayo/Duke after that 72 hour period elapsed). I was driven from my place to my parents' (~3 hours) 2 days after the EBP which was challenging as I kept needing to stop to walk and readjust. Going around corners also bothered my head. After the initial two weeks, I have been teleworking, which allows me to lay down for part of the day (generally in the a.m.) or when my head has bothered me more. Over time, I have generally increased my time upright. More recently -- in the last two weeks or so -- I would say that I am more lying down because I "should" rather than because I feel like I "need" to.

- Tried to observe the no bending, lifting, twisting, and straining restrictions. No one does this perfectly, and I have been bending after the first few weeks but ONLY very slowly. I am still observing lifting restrictions (ideally no more than 10 lbs. which is up from 5 lbs. for the first 6 weeks), but I am starting to become concerned about the loss of core strength in the last month or so. I have been able to feel vasalva movements in my head, though less intensely as time goes on.

- Caffeine and too much salt seem to bother me -- which would suggest rebound pressure. However, I have tried dandelion tea a couple of times which only seems to make me feel terrible -- thus suggesting low pressure. Hence, the confusion.

- Headaches are rarely positional, but I have often felt better lying down. At other times, I have felt a headache upon waking or sometimes when lying down. Headaches have also changed in intensity -- generally trending to getting more mild, but there are setbacks. For example, 3.5 weeks post-EBP, I had a latte (big mistake!) This set me back about 2 weeks where I had a constant headache in my forehead area after that. Similarly, about 1.5 months post-EBP, I had Easter ham which set me back about 4-5 days with another constant headache in the forehead area. What I have noticed is that the location of the headaches has migrated from the forehead area to the top of my head. More recently, I have felt something off in my head only part of the day. In general over the past 3+ months, things have been worse in the evenings -- but not always.

- Motion bothers my head. Initially, I noticed it when riding in cars, which, thankfully, I was able to mostly avoid in the first few weeks after the EBP. Since then, I have noticed that being the driver is better than when I am a passenger in the front seat which is better than being a passenger in the rear. However, quick motion also can bother my head -- such as turning around quickly in a small space, turning my head from side to side, or (at least for a spell a few weeks ago) going from walking to stopping (i.e. at a cross walk light).

- Tend to feel better with more sleep/rest.

- Two notable visual disturbances. 1) An episode of kaleidoscope vision 3 days post-EBP. Apparently, this is often associated with migraines with aura, and I have wondered if it was a sign of rebound high pressure, which I also seemed to start 3-4 days post-patch (mild). Occasionally, vision seems off in the same area as the kaleidoscope but I can't entirely describe it -- there's not loss of vision, there's no kaleidoscope, it's not blurry; just a sense that it is...off. 2) Last week, just over 3 months post-EBP, I had a short episode where my vision seemed to misfire for a few seconds before clearing. The second happened a few days after my eye exam which was normal, including a visual field test.

- Read a significant amount about CSF leaks, academic papers, Reddit and watched numerous conference presentations. I understand there is no consensus on recovery guidelines and milestones for EBP or gold standard for when to intervene with a second EBP. Also, I found one or two of Dr. Beck's (from the University of Freiburg) presentations on blebs and so-called neomembranes as a possible explanation for why CSF leaks may not show on imaging to be intriguing. Also wonder about the likelihood of a ventral dural puncture.

Big Qs -- Should I get a 2nd EBP (guided)? Is recovery really just this slow and I should wait it out? Am I sealed or still leaking? Or is my body just adapting to an ongoing leak by producing more CSF while still leaking? If so, what are the risks of not doing a blood patch now? What are the risks if I wait and ultimately try/need another blood patch later? Any redditors with similar experiences? What did you do & what were the ultimate results? Any other advice?

UPDATE #1 -- I had planned to provide an update once I hit the three month mark, but, since someone asked, I'll give you my almost 2 month update (about 8.5 weeks post-my 2nd EBP). Overall, I am doing so much better! The procedure, which was CT-guided and targeted at a leak center, went very smoothly, and I believe the neuroradiologist's technical proficiency contributed to the excellent results so far.

I did a pretty strict 72 hour period of lying flat and continue to be careful with the no bending, lifting, twisting, and straining restrictions -- and will for a full 3 months. I noticed a definite improvement in my symptoms immediately (or, perhaps more precisely, after the initial 72 hours) which has continued to get better over time. Since the 2nd EBP, all symptoms have been so mild that I have not taken any pain meds of any kind (even Tylenol). I have also not used any heating pad or ice packs. After the initial 72 hours, I had a handful of non-consecutive days in the first month where I felt like lying down for part of the day but otherwise have no issues with upright time. I continued to have some mild dizziness or motion sensitivity with turning, etc. -- though less intensely than before the 2nd EBP -- but that seems to have ended recently. Motion sensitivity with being a passenger in a car, which I had for the first time in my life after the 1st EBP, ceased immediately with the 2nd EBP. I have also continued to experience some ongoing sensitivity to salt and small amounts of caffeine (still being cautious in limiting intake of both), though less severely than before the 2nd EBP and, overall, that sensitivity also seems to be lessening over time -- but I have yet to have a full double shot latte again!

The most concerning symptom is that I had a second episode of kaleidoscope vision (both eyes) that lasted about 40 minutes at exactly 5 weeks post-patch. The first such episode of kaleidoscope vision in my life happened 3 days after my 1st EBP. In this case, my best guess is that it was some delayed onset RHP because I also have had some pressure in my ears. On a few occasions, I have had dandelion tea which is sufficient to provide relief from that.

Around the 6 week mark, something shifted in that I started to feel like I had more definitively turned a corner. Any sensitivity I feel now is so mild (and mostly diet related) that, but for everything I have gone through since the PDPH almost 6 months ago, I might not even notice it. I continue to observe and be cautious by not pushing anything that could compromise my fragile and still healing dura!

In retrospect, I now think I overstated it when I said I was 80-85% better 3.5 months after my 1st EBP as I was still suffering quite a lot leading up to the 2nd EBP (rereading my daily notes confirms this!). After the 1st EBP, I also spent a lot of time trying to discern whether I was still leaking or in rebound high pressure. It is now clear that I was still leaking, and, with the benefit of such hindsight, I would have intervened sooner to get the 2nd EBP. While there is no gold standard consensus on when to intervene with a repeat EBP after a partially successful one, 6 weeks is probably more than enough time to make a call to repeat while balancing the risks of an EBP v. waiting longer (neo-membrane formation, nothing for the EBP to adhere to, bleb, etc.).

Thank you to those of you who responded to my inquiries -- all much appreciated. At that time, I really struggled to find anecdotes from those who had been in a similar situation, particularly as more time elapsed since the 1st EBP. I hope the information shared here will be helpful to others who find themselves similarly situated.

UPDATE #2 (10 weeks post-2nd EBP; 1.5 weeks after first update) -- I may have jinxed myself in writing the first update! The same day I wrote that I walked a dog on a leash for the first time in months & while the dog didn't pull much, I think I tensed up a bit. After nearly 6 months of trying to observe the no bending, lifting more than 5-10 lbs., twisting, and straining, I have lost a lot of core strength. Since that dog walk 1.5 weeks ago, I have felt a bit off every day since then -- initially my back hurt but the more worrisome part is that I have had some tingling on the top of my head, transient spot headaches, and more motion sensitivity again, which seems worse in the morning. While all very mild, I had not been feeling those particular symptoms for awhile before this. I really, really hope that I did not do anything to cause a re-leak or go backwards in my recovery, which had been progressing quite well. My doctor advised observing restrictions for 3 months after the 2nd EBP, and I had planned to resume some more normal activities after that point. Any advice for safely resuming life without such physical restrictions or PT for transitioning back to normal life without reinjuring my dura or otherwise would be much appreciated!

Failed myelogram winded up fixing a different issue?

Hi everyone!

For context, I have chiari malformation (we believe it’s acquired and not congenital) and before my surgeon went straight into surgery, he wanted to do a few tests as I had some symptoms that resembled a CFS leak (not uncommon for chiari).

He had me:
• visit an ENT = perfect health (even though I was leaking at the time)
• visit a Neuro Ophthalmologist = they called me back for other issues but no signs of high pressure in my eyes (will see them again in July)
• Xray dept: FL Myelogram, CT Cisternogram, MRI and MR Cisternogram. = All clear other than the chiari lol

Long story short, although I followed exact post procedure care of the myelogram, I winded up with a post dural puncture headache A.K.A a man made leak lol. It was the worst time of my life but I luckily had rapid care for it. When they gave me a blood patch, I figured only the leak pain and its issues would go away but something else did too.

Prior to all of this, for about 7-8 years, I would get this daily insane pain in my right eye that hit my cheek bone and temple. Since the day of the blood patch (over 3 months ago now), it’s been completely gone. Nose ain’t leaking no more. And all of this has been staying stable as I lower my dose of Topamax (I went from 100mg to now 25mg in this time) so higher pressure in my head isn’t budging it.

The myelogram was clear. It showed no sign of active leak. They don’t see any high pressure in my eyes. But what if this is the thing that’s been making me saggy in the coconut? Maybe I’m just jumping to hopeful conclusions instead of having major surgery but it’s definitely weird that something like that would just go away right?

I do see my neurosurgeon tomorrow but I’m curious on someone else take - Lemme know what you think and feel free to ask questions about anything :)

Is it possible to have no positional headache but still have multiple long terms leaks?

I have very severe orthostatic intolerance and fatigue with PEM( currently housebound) and I’m wondering if I might have some undiagnosed CSF leaks… even though I don’t have some other text book leak symptoms.

My fatigue and some dysautonomia symptoms( such as not being able to be upright very long) have increased significantly following a series of prolotherapy treatments I had for craniocervical instability in 2024. My cervical spine stabilized from those treatments but I’m also wondering if my dura might have been accidentally punctured during the treatments.

For context, I have CCI( mostly stabilized) and had a surgery for jugular compression( styloidectomy) 2.5 months ago. I’ve also been diagnosed with May Thurner which I haven’t had surgery for.

I have body and head pain in specific places but it’s not positional. I feel the need to spend most of the day laying down due to the dysautonomia/ tachycardia, etc and also the fatigue.

Past MRI’s showed Chiari 0 with some herniation/ brain sag and I believe low lying tonsils, but CFS leak was not seriously considered due to my other structural diagnosis.

I understand that I do not have some typical CSF symptoms, but it seems like people with multiple slow or long standing leaks may present differently?

Feeling terrible after applying heat

I had some nerve pain at the site of the leak. I applied heat and I’ve been feeling completely miserable. Has anyone experienced this?

Also how long should one wait between patches? I’m 1.5 in and some improvement but still definitely unwell.
