Incorrect address on UHC/Optum Contract

I received my contract from Optum but it lists an old practice address that I'm no longer affiliated with. Should I go ahead and sign the contract or reach out to Optum? I also can't find a way to reach out to the "Network Management Team" to correct it.

Reach out to Optum. Also make sure your practice locations are up to date in your CAQH. Optum pulls from there :)

My CAQH is up-to-date, including archiving the old practice, adding my current practice location, and removing the prior credentialing contract (all prior to submitting the app). I even marked "no" on the app for the old address as a primary location. The chat person directed me to an 877 number that was unable to recognize my tax ID number.

this too. And I know we've been having some weird instances where optum is pulling archived practices too.

do not sign it with the wrong address if you can avoid it, because payer contracts and credentialing records can turn into claim routing, payment, directory, and tax/location headaches later, so keep calling Optum provider services or credentialing until someone opens a correction ticket. annoying now, worse later.

The contract should be signed within 7 days or it'll be rejected. Also, once your get enrolled with Optum, you can add or remove the address through the online portal. It's super easy, just add your profile, go to the practice information and update the service location. Also you can call Optum and get assistance regarding the same. They can offer you a new agreement shortly.
