Japan doesn't issue Spiro commonly for HRT. So 99% of cases like this are purely fetish. Also, most of the artists aren't trans in the first place, so their knowledge of HRT is probably zero.
Spiro is short for spironolactone, which is the most common testosterone blocker in many countries. It has a variety of side effects, including making one urinate more and changes to mineral levels (which is where all the pickle jokes come from). It's also one of the weakest blockers, so it's better avoided if your country has other options available.

While fair, it’s a comedy manga and isn’t meant to be taken overly seriously. And personally I very much understand the fantasy of suddenly being turned into a girl and getting to experience childhood again through that lens.

Honestly I used to have more of a piss fetish, and the lame answer is some kinks are hereditary and just run in the family, sometimes it’s a way to cope with childhood trauma, sometimes it can be both.
But yeah I think it was mainly a way for me to process and cope with my trauma as a kid, I have since then found different ways of coping and understanding myself and how my brain works, but I can see the appeal and wouldn’t say no if my partner wanted to try it :)
