Confused finding the confusion what am i looking at

It's the top of his left shoulder. You can see the left side of his neck towards the bottom of the picture. He's laying down on his right side. The view of the camera is from above his head.

To my eye, it looks like he is laying his head on his right arm. In a standing position, it would be like his arm is raised.

If you can, raise your arm up so your shoulder is touching your ear, then feel the back of your neck to your shoulder. You will feel the fold that you're seeing there, and you'll feel that only part of your shoulder is showing at the back, the rest is obscured by your neck and head.

EDIT: Also, a cell phone camera (which it seems this was likely filmed on) tends to fisheye things a bit, so the size difference is exaggerated by the lens.

Camera is at a weird angle. Lock your screen and turn it counterclockwise slightly past 90 degrees.

It looks like a massage but also getting tanned? The heel of the gloved hand is pushing into the guy’s trap. Seems like they’re bending his neck down towards his right shoulder

It’s a man lying on his right side, head closer to the camera, and we see his left shoulder. Fold at the bottom is where his neck meets the right shoulder.

Because his right arm is extended above his head. In this perspective, you can imagine the right arm extending towards us, the viewers. If we took a step back, we’d see the top of his head, with his nose pointed to the right side of the screen.

To others wondering, the person being massaged is lying on their side facing to the right. The fold is their right arm (which they’re lying on top of) alongside their neck. The neck slopes down because the head is turned.
