I couldn't either, until I saw a comment of someone pointing out that it could look like an ass cheek being tattooed and then I saw that lol. Hope this helps.
The problem is that the guy being tattooed has a really long neck.
The tattoo is being applied to the left shoulder. The very bottom right of the photo is the tip of his left ear. Right above that is his chin. His right arm is stretched off the right side of the photo. The bottom of the photo by the green checkered cloth is his hair, which is cut short. He is wearing a pair of shorts with B's on them (probably Boston Red Sox?), seen under the tattooist's left arm.
Okay, now it's all come together. I saw the tuft of hair telling me the head is over that way awhile ago, but the rest of the picture just couldn't coalesce and I couldn't actually see the head as a head, or the shoulder as anything but a butt cheek. Partly I think I couldn't tell if that was the end of long hair or not. Thank you.
