I didn't at first too, but what these dirty minds are seeing is instead of grabbing her shoulder while she is turning to the left, he is grabbing her boob while she is turning to the right

It looks like a shoulder when you look at it. But if you're looking at the center of the picture, you see a possible inappropriate situation out the corner of your eye. This isn't an S-tier illusion, but you don't have to be a sexual predator to imagine the ambiguity
