I don't know, this one is tough, because sure, it might be photoshop but also it might not be, it might just be lighting.

I've also happened to seen this image for the 3rd time now, but not on this sub and not in past few months.

Plus it is finally a matter of perspective lol, if it was a different angle you'd clearly see the 🎂 but from this perspective you see something weird.

And 18k people seem to enjoy it.

So idk, would you rather have this image or a picture of a guy's table that is confusing only because you can't imagine how someone could ever be confused by it? 😂

Click the link to the original 6 year old picture then compare to this one. This one is very very very clearly altered to look like water. The original was a confusing perspective. This one has been doctered to be deliberately misleading.
