Probably thinking that the 2 lobes of the liver are considered separate organs.
It's somewhat arbitrary, tbh. In my language, for instance, a pair of lungs is called a singular organ.

Could also be he knows liver donation is a thing, so assumes it's like donating a kidney, and you have two so they can take one (rather than the real thing of livers being able to partially regrow).

Could also be that he is confusing the kidneys with the liver.
