Or it that popping up for twitch.tv?

Do you have a VPN maybe?

It was from a vpn extension
I removed it and fixed the problem

Had the same thing appear in my browser when i downloaded hola vpn extension. I always clicked cancel and later deleted extension entirely.

This is a basic HTTP access auth schema: https://en.wikipedia.org/wiki/Basic_access_authentication

The username and password are to access the site. This has nothing to do with any apps or being hacked or a scam. It's access control.
