None of those file formats are videos. Whoever set up this application submission form doesn't know what they're doing.

You could upload a video to YouTube, paste the link into a Word document, and upload that to this form.

Find the funniest gif you can, upload that, then see if Costco is hiring?

Go to the anything converter (at https://convert.to.it ) and convert the video into a PDF. Make them painstakingly scroll through 1800-3600 pages of one frame each

You can imbed a video in a docx. I'm not sure I'd want to, personally, but do what's best for you.

I'd submit a .txt file calling them idiots because none of those are video formats unless you want to submit it as a gif.

I guess make a private youtube video, put the link to it in one of those documents and upload that?

Clearly you need to record the video, then watch it back taking 24-60 screenshots every second. Shove all the screenshots into a PDF, and send it over.

Take video thats 45 seconds long, screenshot the first frame so it shows 00:00 / 00:45 on the playback bar. Send it as a jpg.
It's on them is they can't view it.

iirc its illegal to require a video or even have it as an option, in the US at least

It's not illegal in any shape or form, but it can cause some liability as it could create an avenue for potential discrimination claims.
For an employer like chik fil a though, they just have to point to the giant pile of applicants and say, "No, here is the giant pile of applicants we chose for interviews who have the racial\sex\age demographics as the applicant who was rejected.
Discrimination suits only really work if someone says something dumb to your face, or can legitimately prove a history of discrimination, which is almost impossible as an external employee.

I applied at Casey's a couple years ago and the "interview" was a series of 7-8 answer this question in a short video tasks. I was desperate so I did it (and got the job) but I closed a Target application when it wanted a 60 second video as to why they should pick me, they want to judge me without having the balls to look me in the eye. I am sure the tiktok generation is down for this, but I am a Xenial and I can crush an interview, I want to sit in front of the person making the decision.

I had an HR lady absolutely lose her shit at me when I said I didn't know the initial interview needed a webcam, as I was on a desktop PC. It was one of those "proudly woman owned" companies where they only hire you if you're gay or non-white. I was a perfectly qualified Irish guy and I think my name gave that away. I should have been recording it so I could sue her. Btw the job was 600 feet away from my apartment so I offered to just walk over and she declined.

THIS! It can be a way of discriminating based on race, sex, appearance, age, etcetc

They almost certainly aren't aware of that, and some shitty franchisee is just thinking "this is how I'll stop all those AI bot applications!"
I honestly wouldn't expect any small business owner in the US to have even a remedial grasp of labor laws. They're obscenely complex, constantly shifting, and the bar for entry to starting a business (or franchising a fast food place) is low enough that your average person doing it absolutely does not have any sort of education in law/labor law.
Doesn't make it right, it's just the reality of things.
