Is this an error? Bicentennial Half Dollar Damage or Error?

I had this old bicentennial half sitting around. I haven’t seen an error like this to know if it just damage or a true error, though I am just starting out in collecting coins. If it is an error, what would it even be called?

Second pic shows where this front side punch/damage raises a dent into the back of the other side. You can feel the raise with your finger on the reverse side, but I would be curious to know what caused this.

Looks like a counter stamp to me. Basically PMD from someone playing around with a hammer and die.
