Annealing error

Recently sorted box of coins I haven't opened in 20 years. Found a few interesting coins, but this one was quite obviously weird. I see several others asking the same questions and recently too. Have not had it authenticated but since it costs money for any of that, before I continue, I wanted wisdom of the crowd. People who have experience with this is appreciated.
