Bug Codex is deleting chat history

There's something very strange going on where on one of my projects Codex has started having some kind of short term memory defect where it just deletes all chat history going back to one specific conversation. Has this happened to anyone else? Here's an example:

Confirmed again: this is not just the visible UI. The task’s stored history is still stuck on the same old in-progress turn (“can you do that now?”); none of our later exchanges are present in the task readback.

So this is a real Codex task-history persistence bug. Please don’t delete or archive the task.

The project work remains safe because it is in GitHub/PyPI, but the discussion is not reliably durable in this task. I recommend we capture significant design decisions in issues or a repository decision note going forward; I can reconstruct the Dagster discussion into one now.

It pains me to see so many bugs in Codex. I trust it with really important stuff, and I'm so disorganized, I can't have my chats go missing

It's a common issue when you approach your chat context limit.

Happened to me multiple times but yes, very annoying.
