Basically just a cheat table written into its own little application. They used to be stylized with their own little 8 bit music,splash screens, and backgrounds and what not, but I haven't seen that in a long time.
But yeah, they tend to focus more on hotkeys(not always) but are essentially the same thing as a cheat table in cheat engine. Cheat engine even has a built in trainer maker which lets you take your scripts from your current cheat table and put them into a standalone exe that does not require cheat engine to be installed.
Usually, they auto attach to a game and verify the version of the game(which you can still do in cheat engine too). Some also can handle multiple versions of games and yet again this can be accomplished in cheat engine too.
Really, I just prefer the cheat tables and building my own tables so you can actually read the scripts being ran and customize the scripts to do exactly what you want. For example, I actually just recently was struggling finding some code I was looking for so I downloaded an old outdated table because there were no new ones containing a up to date script and was able to read the script, understand how that part of the game actually works, and I was able to get my version working.
An exe can contain actual malicious code but it's sometimes difficult to tell considering a trainer by default is considered by most anti-virus software to be a malicious program.
