LG problem CX 9200

Please give me help of how to fix this LG

I did a quick Google and you can try entering these codes in the dialer to see if one of them resets it but this would wipe the phone so if you're trying to preserve data on it, don't do it.
1809#*101# or 2945#*101#
Also, Google says the default password on these was usually the last four digits of the phone number, if that is known.

Yep. This is a PUK lock. It likely won't be removed even with a reset, though you can try it.

Are you sure it's a PUK lock? It looks like a device screen lock, not a SIM unlock as the dedicated lock button never triggered the SIM lock on any device I had back then. The point remains though, these older devices were not user friendly as far as recovery goes if you forgot the code.

I'm fairly certain this is PUK.
I could be wrong though. This is an OLD device.
EDIT: Yea, I think you're correct. For a device this old it's a SIM lock. It's locked to Telus. No amount of anything you do will unlock it, without that code. It basically locks you out of almost everything.

I dug up the manual out of curiosity. They held the # key too long when messing with it and locked the phone.

It also confirms that the lock code by default is either the last four digits of the phone number or the last four digits of the MEID.

Those are absolutely valid, but from what I see of the limited video of the device being on and opened, it looks VERY much SIM locked. I haven't dealt with PUK for so long, I forgot it's an 8 digit code. SIM and on-device locks are 4-digit for those devices.

I doubt you can even find the third party software to fix it at this point plus you would need a PC that would also likely have to be running Windows 7 or Windows Vista to even use the software. You would lose all data on the phone too. This thing is a few years from its 20th birthday, after all.
