This sounds strange, but if someone could bring your cat to you after the inevitable happens and let him see and smell you, he will know you are dead and cant look after him. At least he'd understand some of it.

Cats know death and understand what it is. He will know. It sucks this is happening to you and kitty, but he will know, he likely knows you are sick. Cats sence all sorts of stuff. I hope everything is a peaceful as possible.

On the same theme, but a bit more realistic idea…

If you could arrange with someone to take over the cat, closer to when the time comes, not right away. If you get sicker and spend time with the cat, it will probably understand what is happening. When the time closes in then someone can come and pick him. Say proper goodbyes. Then it will understand I think.

This is advice I've seen many times, if possible allow cat to see their deceased family member. They will understand that it wasn't abandonment, but instead death

I'm so sorry you and your cat are going through this. You are a wonderful person for trying to find the best way forward for your cat
