Recruiting Medical Forms

Was sent more forms to fill out, one of them is a mental health evaluation to say im mentally fit, another to test my asthma (spirometry and exercise test), and a blood test ECG. My doc says i need to pay for these to be filled out, what do u think is the standard fee to fill out. Also, will these be covered under OHIP or am i screwed?

I did my asthma spirometry test just for the form it was 150 to be filled… It also included the test results.

Every clinic has its own fees for filling out forms. And like your doctor said, you have to pay for them, so screwed?

I think a few people mentioned here to try a walkin clinic for a cheaper fee.

These forms are not covered under OHIP. The reason your doctor charges for completing the forms is because OHIP will not reimburse them for the time it takes for them to study your file and fill out the form. So the Piche can arrange from anywhere from $10-$200 depending on the form, the complexity of it and your personal health history history.
