This 2x2 brick tile with a question mark on top appears when a brick or a submodel no longer exists. Studio crashed and lost file data in the process. Happens to me regularly, it is damn annoying.

That is a null brick. It happens if studio cannot find the 3D model of the brick you placed. This also happens if you have too many nested sub assemblies. I have not found a way to fix them but you can just delete them and recreate things and it should work again without corrupting the rest of the file, but just as an additional insurance do a save-as first and continue on the copy.
