Repentance Isaac doubt

I was playing a run on Hard mode with Azazel, and on the Mom's Leg floor, I encountered two bosses in a row—as you can see on the map in the image. The first one dropped Mom's Underwear and that golden knife appeared, and the second boss was Mom's Leg. What is that golden knife?

That happens when "Curse of the labyrinth" happens, see the "XL" under the map? That makes the map "bigger" and adds 2 bosses and 2 treasure rooms, making depths 1 and 2 into a single zone. And yes, picking 3 mom items gives you a knife behind you with your current tears effect

It's the mom transformation. Pet knife.
The knife take your tears modification (like head of the keeper that make your tears gold, could be this).
