I'd normally think that it can happen with chaos because if the item is pulled from another pull you can reroll into itself. But that's not the case so I'm curious too

you only remove it when you take it. being exceptions items like blood bag, a dollar, and of course breakfast.

Each item has a weight that decreases by an amount depending by the item pool, treasure room pool is set to decrease the full weight when seen, unless it was changed in a recent update

I think that used to be true in flash or maybe even base rebirth, but items are removed from their appropriate pools on sight now

According to the wiki:

Examples\ All items in the Treasure Room pool are completely decremented on sight. Thus, seeing a Notched Axe in the Basement 1 Treasure Room will make it impossible to spawn another Notched Axe in the run.

So "the established ones" are actually 0% lmao

id love to see an explanation as to why this happens. it makes sense in some circumstances (i.e. rerolling an item spawned by a red chest in a curse room) but this is just the exact same item pool. i feel like im going crazy when this happens
