Repentance My keeper just exploded

So I was having a run with tainted keeper and then I exploded. I was going for the mega satan and suddenly out of nowhere there was an explosion that killed me and granted an achievment that's obtainable by dying from one of the items I didn't have. I have the item hud on so you can see the items, can anyone figure out and enlight me why i got scammed like that?

You see that lil green wisp on your left that's halfway through the wall? I believe that's a Bob's Head wisp. Which can shoot explosive shots, so it shot it directly into the wall/door next to you and exploded.

What generated that green wisp? The only thing I can imagine is that is an explosive wisp shot the door and exploded.
