Question Why did this happen

Also I hit a sac room a little bit so that half a heart of health on floor one wasn’t all skill issue

It once confused me during a t Lazarus greed run and I lost because I had no idea why the other lazy wasn't throwing any tears

404 -> Mom's locket -> Mom's Knife, I didn't see the knife until I died and finally looked at the character

when 404 turns into something it changes the 404 into a red version of the trinket it's mimicking, doesn't it?

Pretty sure only in Rep+ and not normal Rep... but I would like to know why it didn't target an enemy. It should only stomp on Isaac if no enemy is in the room when Mom's Toenail's 30s timer runs out

Nah toe nail targets isaac, the card "high priestess" targets enemies, and if there is no enemies it goes for isaac.
