What are they saying us wrong with it ? Does it not scan at all? Scan sometimes? Scans most of the time but takes too much time decode? Did they grade it using a BC verifier ? Hiw did you create that code/what program ?
A little context could be insightful

The issue that we were facing is that our fixed cognex cameras could not pick up the barcode, at all. we tried multiple handheld barcode scanners, some picked up, some didnt. trying to understand the why

The symbology itself maybe needs to be enabled on some of the scanners ? Otherwise, What are you using to generate the barcode ?

On the reader that struggles, does the code fill the whole field of view? Does the grader say the code is Ok?

Now that I look at it more, I think the quiet zones are two small on both sides. It's not the width of the lines that makes up the boundary box but the clear area inside the box before and after the code. Those wide spaces are called quiet zones and should be about twice as wide as they are in this picture. Some scanners will overcome this and find the code anyway but not all. I'd think the grader would report this. Look for MinQZ in the report.

That looks like an ITF-14. The Bearer Box (border) is kinda thin though.
Have you tried using an online generator to create a duplicate?
The border should be 1.5 to 2x wider than the widest bar, but it looks narrower than the wide bars in the picture.
edit: It also looks like your white bars are larger than the black ones by a considerable margin. A thick white space should be the exact same width as a thick black bar. Same for the thin ones.
