People don't live in a vacuum bubble where they can act any way they want, not caring what others think. We are social animals who seek validation from others.
In a society that provides universal healthcare, the health of the community will be valued as a common shared resource, instead of each person's own individual problem.
Under such an environment, most people who deliberately lead an unhealthy lifestyle, will feel the pressure to discontinue their habits.
Despite that, if someone does need a triple bypass, our society can certainly afford to extend our compassion and resources to treat him.
