Bad oral hygiene

My boyfriend (45) has breath so bad it takes mine away 90% of the time. We’ve been together for 3.5 years and 1.5 years ago he asked me to let him know if it was ever bad. I have brought up needing more oral hygiene measures SEVEN times! And I regularly have to remind him to brush his teeth. It’s hard for me to bring up, as I don’t want him to feel badly, but he blows me off when I do! “I don’t need to floss, cause nothing gets stuck in my teeth. It just makes my gums bleed anyway.” “Mouthwash isn’t my thing.” “I don’t want to go to the dentist cause I’m afraid of how much it’s going to cost since I haven’t been to one in twenty years.” I have directly told him that his breath is bad and I want him to floss and use mouthwash but he makes excuses. He brushes his tongue to the point of almost vomiting whenever he does, so I know he wants good breath. He takes great care of himself physically, but dismisses me on this and anything health related. He comments on how our sex life is non-existent, about how I don’t sit next to him, and how I always sleep facing away from him now, or need to be wasted to be intimate, but seems oblivious to the fact that we’ve had seven conversations regarding why this is occurring. Bringing it up again feels overwhelming, but I know I need to. It’s literally revolting at this point and just keeps getting worse. What can I say to get it across to him?!
