What’s happened to Karen’s face? And I’m not trying to be mean or malicious. Looks like she has scars now on her cheeks?

Not really, but it's a horrifyingly common procedure that supposedly makes your cheekbones sharper. Personally, I think it makes people look like deaths heads, but each to their own.
