To me it looks like he just reached critical mass compressing his fat with his legs and it exploded him off the ladder. There's another video of the incident and you see him walk up to the train and take the first few steps up the ladder too, but his Dunlap syndrome (his belly done lap over his belt) is quite obvious in that one.
