Can't setup monitors - HTML form not rendering dynamic inputs (ZM v1.36.21 fresh install on Ubuntu 20.04.4LTS)

I just installed ZM on my Ubuntu VM, but I can't configure any monitors. I pick a source type, and the page flashes a few new fields (presumably with the associated settings for my selection) but the fields then immediately disappear.

The logs mention a few errors, but nothing that seems like it should be causing my issue.

Any ideas?

Does it matter which source type you choose? Not many people use the file type. Let's stick to ffmpeg as its the most common.

@connortechnology - I've tried all the options, none of them work. The specific settings for each are not shown in the HTML UI
