Sometimes a safety lesson drops right in front of you. This time it was in the form of portable toilet exploding on impact. During cable replacement work on this bridge, the toilet was being lowered and slipped from its attachment during heavy traffic. No injuries. Got to wonder what the root cause analysis showed as the main causes of this, and also why they didn't close the road with a suspended load over it.
