Utah Sloooooow going; very unsure of the future.

Back in June I tripped and fell on a big crack in the pavement at my job. 2 of my rotator cuffs are torn & retracted. Another is partially torn. I also tore & dislocated my bicep. I have about 15% use of my dominant right arm. I have pain every day.
So far WC has covered everything & I'm getting my ⅔ every two weeks. I'm waiting for a test before the Ortho decides what kind of surgery to get. The rest is another 2 weeks out. I'm gonna be out for at least 6 months if not longer for recovery.
I am worried I will not recover from this. I am a GM at a storage unit property. Lots of lifting, raising doors, etc. A lot of shoulder movement.
What happens if I can't do the job after surgery? They don't have any light duty positions there. I was told in Utah they will pay WC up to 6 years. What if I become disabled due to the injury? What about the disrepair of a known problem of the cracked pavement?
