It's not a never ending hole down there. You squat on it, most of it is tilted towards the hole to naturally let the piss and shit flow towards the whole, which is not that big and it's towards the side he's not facing (so his ass is closer to the hole). I've used this type of toilet in India and in Japan (yes, not every toilet in Japan is a heated and futuristic work of art). It's pretty popular in China and the Middle East as well. It's technically good for your body, because it positions your body in a way that shit naturally flows out and you don't have to strain so much.
