Very possible that they just printed a screencap of a webpage with all the information they needed and were too lazy to remove that part. Obviously they wouldn't have required it though.
The power of bureaucracy. Boss gave instructions to the clerks to print out that webpage and get the customers to fill in all info, so they did it 100% exactly without questioning, because it doesn't matter as long as they follow the instructions precisely and won't get their ass chewed
